Using the Form Designer allows each customer to customise what information they want to collect at the point of referral, and which of the fields on the form are mandatory.
All referral Forms must have a Service User Details Section and an Address Section, but the validation on fields within these sections can be relaxed. A Surname is always required, and Organisation, Referral Date, Priority and Scope will always be part of the form and are mandatory.
When selecting add Referral, the user will be offered a choice of available referrals, based on the logged in users scope.
Any pre-defined sections used in the referral form will populate the appropriate place when the Service Agreement is created eg. Dwelling Characteristics and Keywords
Any custom questions added will be retained for reference in the referral.
Note: If a referral is completed without title, forename, DOB or address when the Service Agreement is edited, the validation will still apply.
