Expected Outcomes

The expected outcomes can be supplied to the Service provider so that service provider and referrer are clear on what the expected outcomes are. This will allow the Service Provider to detail what the expected business benefit is when the service is deployed, so that a cost saving can be attributed to the service.

These need to be defined in the configuration file before they are supplied within the assessment section of the Service Order, specifically in the section <xs:element name="expectedOutcome" type="xs:string"

The default values within the application are:

To reduce carer strain
Prevent/reduce frequency of hospital admission
Delay/prevent admission to a care home
To promote independence?
To improve safety in the home?
Minimise consequences of falls?
To manage anxiety and improve confidence?
To reduce strain on carer or carers?
To reduce or prevent increase in an existing care package?
Prevent and/or reduce frequency of hospital admission?
Delay and/or prevent admission to a care home?
Reduce Homecare interventions?
