Create New Template

To create a new template


Note: The scope of the template will determine which Service Agreement/User can use this form when creating a new assessment, referral or task. You will notice the revision for the new template is Revision 1.

Note: At any stage the Form can be saved in draft format and edited later, draft templates can never be selected within the application


Sections

Add New Custom Sections


Repeat this process until all the Sections are added and complete and then start to add the questions within the sections


Add New Pre-Defined Sections


If creating a Referral Form Only


Note: Pre-defined Sections cannot be edited, apart from the Section name.

Note: You can only have one of each type of Pre-defined section per Form Template, except for Contacts and Services where multiple are allowed

Note: Contact and Services Sections can be marked as Required, un-selecting the tick will make them Optional
