Creating a Service Agreement with a Matching Address and Person

• Once mandatory fields have been completed on the new referral
• Select the [Validate]
• The system will then search existing Service Agreements and display any potential matches
• If an address match (postcode) and person match (DOB) is found by Service Manager, the system will display a 'both' match which will be sorted to the top of the validation page.  
• Select the record by clicking the Match Icon next to the appropriate Service Agreement
• Select [Match]
• A Change request will be created on the existing Service Users Service Agreement

Note: There is the option to create a new Service Agreement, even is a person and address match is found, i.e. the Service User has two dwellings and two sets of equipment, in this option you would be correct to create a second Service Agreement for the same Service User.

Typically a service user has been re-referred into the Service when they are already an existing Service User.  You cannot match to a terminated Service Agreement
