An installation request is always instigated through Make a Referral and is created when a referral is validated.
A Change request can also be initiated through the Make a referral and the validation process, however it is also possible to create a Change Request by

A Decommission request will also be initiated on the Service Agreement by;

Note: A Request cannot be created on a Service Agreement when there is already a pending request on a linked Service Agreement. [Button will be disabled]
Requests can be initiated from PNC using quick short-cuts, these short-cuts will launch the Service Manager pages depending on a request being in process or not.
